Program Overview

This program enables individuals to generate a unique QR code (or link) to share within their online networks. Participants, referred to as "Referrers," will earn a commission for each confirmed enrollment originating from their referred leads.

Defining a Referral

A referral is any individual who starts their health insurance application by submitting the initial intake form provided by the referrer. These referrals become eligible for commission once Lyve Health verifies their successful enrollment.

Rewards and Incentives

For each successful enrollment, the referrer will receive a $50 pre-paid Visa card via email. These rewards are subject to a 14-day withholding period to confirm successful enrollment.

Privacy Policy

General Terms

Program Duration and Termination

Violations and Enforcement

12.1. Suspected violations may result in the cancellation of benefits.

12.2. Participation restrictions apply to current/former employees, contractors, or affiliates.

12.3. Compliance with all applicable laws and regulations is mandatory.

Disclaimer and Indemnification

Dispute Resolution
